The MDS Coordinator duties include overseeing and coordinating the Minimum Data Set (MDS) assessment process and ensuring accurate and timely completion.
Responsibilities also involve collaborating with healthcare professionals to develop and implement individualized care plans based on resident needs.
Qualifications:
Bachelor's degree or greater in a relevant healthcare field from an accredited college Current, unencumbered license in a healthcare discipline (RN or LVN preferred) Experience in Long-Term Care (LTC) or Skilled Nursing Facility (SNF) settings Knowledge of the MDS assessment process and regulations Perkins Country Manor is an equal opportunity employer.
